Before your first visit
The first appointment is long by design. Coming in prepared means we spend it on you rather than on paperwork.
What to bring
- A list of every medication and supplement you take, with doses.
- Any laboratory results from the last two years, even the ones you were told were normal — especially those.
- A short timeline: when the symptoms started and what was happening in your life at the time.
- Your insurance card, so the office can tell you what is covered.
The office will email your intake paperwork once your first visit is booked, so you can fill it in before you come in.
